To file a claim for a shipment that we sent to you, please email us by clicking here. Please be sure to provide the following information to ensure the claim is filed properly. Once the claim information is submitted we cannot change it so please be thorough and check your responses for accuracy.
1. Provide your MTO Battery order number.
2. Please provide photos of your shipment if it is damaged. This would include photos of all six sides of the box.
3. Did you notate the damage with the driver when they delivered the package?
4. Provide the dollar amount that we should request for the shipping claim.
4A. Please note that the requested dollar amount cannot exceed the insurance amount that you purchased. If you are asking for more than the MTO Battery invoice amount, you will need to explain why and provide an invoice or quotation to justify the dollar amount that is being requested for the insurance company to consider.
